Kill an elite or boss that has dealt you no damage. The check belongs to that individual enemy. Damage from its companions does not invalidate it, and ending at full life does not erase a hit it landed earlier.

A roaming pack leader

Finish the Hollow Fields main route, then seek a Ledger Pack Leader with its accompanying hands. The successful attempt in the film comes after the Old Sexton, with the Huntsman at level 11. The packs roam: use their nameplates, rather than expecting the same position.
Put the hands down first while avoiding the leader's attacks. When the leader begins its windup, walk out of reach or roll away; spend your ranged attacks between its attempts to close in.
Keep space between yourself and the leader while taking down its ward and life. The successful filmed fight runs from about 13:17 to 13:58. Healing or drinking is allowed, but neither cancels damage the leader has already dealt.
Kill it and watch for No Deduction. If it touched you, try another leader: the film includes three earlier attempts that did not qualify.

No Deduction recorded when the Ledger Pack Leader dies, and level 12 reached — This leader's death earns No Deduction and takes the Huntsman to level 12.

What goes wrong

One hit from the leader. Healing afterwards does not restore a clean attempt.
Standing still to finish a cast. Its next swing matters more than one extra shot. Step or roll clear, then resume firing.
Confusing this with a deathless region. A Region Without a Deduction allows damage but forbids going down before the region boss dies. This achievement allows other enemies to hurt you, but forbids damage from the elite you are trying to settle.
